Silk syndrome is not an independent disease, but a set of neuropsychiatric symptoms that appear in the afternoon or at dusk in patients with dementia. Although its causes are not completely clarified, various factors that contribute to their appearance have been identified:

  • Alterations in the biological clock: The brain damage associated with dementia affects the suprachiamatic nucleus of the hypothalamus, responsible for the regulation of the sleep-vigilia cycle.
  • Decreased natural light: Reduction of exposure to sunlight can alter melatonin production, a crucial hormone for circadian rhythm.
  • Fatigue and sensory overload: The accumulated fatigue during the day can increase irritability and difficulty processing environmental stimuli.
  • Psychological and environmental factors: Anxiety, stress, noise and changes in the environment can exacerbate symptoms.

Main symptoms

Silk syndrome is characterized by behavioral and emotional changes that intensify at the end of the afternoon. Some of the most common symptoms include:

  • Confusion and disorientation.
  • Anxiety and fear without apparent cause.
  • Agitation and aggressiveness.
  • Handling or attempt to escape.
  • Screams, crying or paranoia.
  • Sleep alterations and night rest.

Impact on the caregiver and the family environment

Caregivers, whether family or professionals, may experience high levels of stress due to the intensity and frequency of sunset syndrome. The Lack of night rest, emotional wear and frustration can generate physical and mental exhaustionwhat is known as “caregiver stress.” This wear can lead to a decrease in the quality of care and affect the health of the caregiver itself.

Strategies for Management and Treatment

While there is no definitive cure for sunset, various strategies can help reduce its impact and improve the quality of both the patient and the caregiver:

Environmental regulation

  • Maintain adequate lighting In the afternoon to reduce the perception of shadows and avoid disorientation.
  • Minimize noise and stressful stimuli in the hours before the night.
  • Create a structured routine to provide stability and patient safety.

Behavioral interventions

  • Avoid caffeine consumption and sugars in the afternoon so as not to alter the sleep-vigilia cycle.
  • Promote relaxing activitieslike listening to soft music or reading.
  • Do not force the patient to sleepbut to allow a comfortable and quiet environment for rest.

Pharmacological support

In severe cases, doctors can consider the use of drugs such as:

  • Melatonin to improve the regulation of the circadian cycle.
  • Anxiolytic or antipsychotic low dose (only under strict medical supervision) to control extreme agitation.
  • Colinesterase inhibitors, used in Alzheimer’s disease, can improve cognitive and behavioral symptoms.

Caregiver care

  • It is essential that the caregiver has rest spaces and emotional support.
  • Find support groups o Psychological therapy can be key to handling stress.
  • Distribute care responsibilities among several relatives or hire professional help if possible.

Silk syndrome represents a challenge in the management of dementia, but with adequate strategies it is possible to reduce its impact. The combination of a controlled environment, behavioral management techniques and, in some cases, pharmacological support, can improve the quality of life of the patient and the caregiver. Education and support are key to face this challenge with greater effectiveness and empathy.
